I say bending over... although by the "cough" part I mean the two tests. The actual coughing part isn't too bad, but the other test they do before or after it is worse.
 
How about, "Please put your feet in the stirrups."

ETA - Maybe I should have said that you are a man and have no idea what you're really talking about. :laugh:
