A guy was born without eyelids, so when they circumcized him they used the leftover skin to make eyelids...hence the term cock-eyed. ;)

Hell, I really don't know.

It's named after chickens which have eyes which don't always point in the same direction. The male chicken is called a Cock.

I'll hazard a guess that since cocks can move their eyes independently that such asymmetrical condition is considered cock-eyed; perhaps made more so by cocks' tendency to jerk head.
