Depends on the restaurant. Probably no jeans/sneakers and your shirt must have a collar. Those are the rules at a couple of the local golf courses anyway.

Since it's not specific I wouldn't assume you need a jacket and tie; it sounds like they just don't want people dressing like slobs. No jeans, no t-shirts, no polo shirts, no sneakers.

If it just says "proper attire" required, I'd go with a pair of slacks and a button-down collared shirt. Maybe throw a sport-coat on too.

I'd assume I was eating somewhere else. If a restaurant requires more than a sweater and khakis, I don't eat there.

Probably just business casual, but it doesn't hurt to be overdressed.
When in doubt, jacket and tie.

I saw one of those signs outside a Mervyns before it closed down. I couldn't help but think, if you had no shirt and no shoes, wouldn't that be the store to go to if you wanted to get some?

No more than a polo shirt and a clean pair of jeans. If they need you to be better dressed than that you won't find me there.

Sport coat and tie would probably fit the requirements. Might even be able to ditch the tie.
