Supply chain breakdown with mixed natural disaster/economic causes. A huge hurricane here, a massive earthquake there, a bad tornado season, and maybe a volcano all in a tight timeline... freeways get destroyed, food production suffers, federal aid is overwhelmed. Food deliveries will grind to a halt, water will become unavailable in areas that depend on having it piped in, sewage treatment and garbage pickup will stop, etc.

Large cities will be thrown into chaos, with riots first, starvation second and waste-related disease third. Smaller towns will probably last a bit longer because they won't have the immediate riot response, but will eventually succumb to dwindling food supplies. Rural food producing areas will survive best because of their immediate access to the necessities of life and the smaller population to put no overtaxing demands on city systems.

The world will be just fine. Unless we collide with another planet when the Andromeda and Milky Way galaxies merge or unless we get sucked into a black hole the world will survive and prosper. Even if we set off every nuke simultaneously all we're going to do is exterminate ourselves and most of the higher life forms. The lower life forms will grow and evolve and restock the planet and we won't be missed at all.
