I have you beat by a slight margin. I had the Stealth 64 Video with 2MB VRAM. It was the thang for Doom II, with my Pentium 90.
 
Some Orchid -EGA- card to replace the CGA card in the first PC I owned. Bard's Tale was rockin' in 640x350!!!

First vid card was a 2MB ATi Rage 3D - had that a while then got a 6MB 3dfx voodoo 1 based Canopus 3D passthru card. I can still remember firing up GLQuake and thinking holy f**k! It was awesome.
 
what was the video card in the C64? that was my first 🙂 after that, whatever was in my 286... supported EGA 🙂

First one I actually bought? As opposed to whatever came with a machine...

Diamond Edge3d. Had Saturn controller connectors, onboard audio and really crappy video.

Next one: A Riva128 4megger. Got that one at BestBuy for $45 open box deal (the new ones were over $200). I think it was an employee who set that one up for himself, but I found it buried on the shelf. LoL

My first card, bought to upgrade my computer, and play a game, was a 16meg TNT2 Vanta pci card. Upgraded from onboard graphics. Woot!
 
I think the first vid card I ever purchased SEPARATELY (e.g. not built into the 8086/286) was a Tseng Labs ET3000 chipset card with 1MB. Hey, it was a Windows Accelerator! 😉

3DFX Voodoo 2 was my first video purchase. Still remember how I saw the Quake 2 in Open GL for the first time, mmmmm

First was Tandy TGA (320x240 with 16 colors!)
Second was Trident 8900 I think (1MB video woohoo)
 
Number 9 771 4mb PCI in 1995. (passed on to my uncle and still going strong)
Voodoo 3 16mb PCI in 2000 (still in second system)
ATI Radeon 64mb VIVO in 2002 (current system)
 
I had a Compusa bought CENDYNE!!!! Pentium 60. First pentium ever. This this cost like $3500 and was used for tie fighter.

Think it came with a
trident 4mb card (p60)
voodoo3 3000 agp (pIII600)
Geforce3 (p4 1.8)
Radeon 9700np (xp2400+)
 
Western Digital 9030(IIRC) integrated on the motherboard of a DEC 486SX25MHz. Oh the memories.
