S3 Virge DX
- card was laying on table, dropped something heavy on it

Leadtek WinFast A250 GF4 Ti4400
- PSU went bad and fried it (along with my very first DVDR/W - a LiteOn 4x)
- kinda *sad face* on this and the LiteOn - used them for years in my main rig and kinda developed an attachment to them :S

8800 GTS (640 MB) and 8800 Ultra, both shortly after one year, suspect bump-gate.
I guess I got really lucky with the G80. Have 2x 8800GTS 320MBs and a 8800GTX, all still purring along nicely. PNY-branded no less :p

Had a 7900gs that started barfing artifacts in 3d mode. But that's the only one I've had die. (had a couple others that were defective from the outset, though.)

Hmm... I think my old ATI 9800 Pro died on me a few years back, other than that I've been pretty lucky.

I can honestly say none. I bought an x800 XT PE off the FSFT forums once and it artifacted a lot but then I saw that the old owner tried putting the stock fan on again and didnt mount it correctly.

6800GT "died" on me because my guitar case fell on it and cracked the core. I also had two GTX 295's fail randomly.

EVGA 7600gt died twice in five years. The last time they replaced it with a 9600gt, not a bad upgrade :)
