what to include when sending a tax return

i used Tax Freedom to electronically file my state and federal income tax returns but there is a little paperwork that must be sent in. however, the instructions are somewhat confusing as to what to include in the envelope (for state & federal returns). anyone file their taxes online using Tax Freedom?
 
For most filings, you're probably just looking at any applicable schedules you've filled out, plus the appropriate (federal, state) copy of your W-2.

And, of course, the check, assuming you owe.
