Acrobat Reader users can fill in PDF forms, but they cannot create or modify the design of PDF forms. To create or modify PDF forms, use Acrobat Exchange 3.01 or later, which is included in the Adobe Acrobat 3.0 package. For more information, see the Adobe Web site (http://www.adobe.com/store/products/acrobat.html).
For details on creating and modifying forms in Acrobat Exchange 3.01 or later, install the Acrobat Forms Author Plug-in 3.5 Update, then start Exchange and choose Help > Forms > Forms Online Guide.
