I have 1164 unread emails, probably 99% are spam.

What do you think I should do? Just say heck with them all and delete them all or go painstakingly slow through each one to make sure if it's real or not.

By the way, I do have a spam detector (SpamPal), but for some reason a lot of them still get through.... doh.

I had about 4000 in my spam folder (marked as Spam by my ISP). Way too many to go through carefully and their spam blocker is good about not marking real mail, so I do a very quick skim and then dump it all.
 
I've gone through 100,000 before (sort by subject, and bulk delete though) so i think you should go through them.
 
You'd better read them all twice.
Then, instead of deleting them, just put them in an archive folder and periodically review it just to make SURE you don't need any of them.
After you have been going through this process for 6 months, you can begin the deletion of them.
