what is this cable called?

In reviewing this page, I believe that cable is the power cable for the floppy drive. It does not come with a USB cable - you provide that.

That's a single USB header (you usually see them as 9 pin blocks for 2 ports, but this is a 4 pin block for 1 port).

awesome, now we're getting somewhere. searching for usb header's on outpost.com, i just see literally the plastic header, not the extension cable i wanted (i'm no electrical engineer), anybody know what the name of the header + cable + opposite header w/ pins is called?
 
1x4 motherboard USB header cable (or variation thereof)

Here is a 16-inch 2x5 header cable that can be pulled apart to make two 1x5 cables:

2x5 USB HEADER CABLE 16 inch - $5.00 plus shipping (and tax if applicable)

Obviously, you would just use 4 of the pins/wires, and you may have to rig the cable to the card reader's internal pin header. I've ordered several of these cables and they are very high quality.
