You could probably just grab something like a 9800 Pro and get that funky heatsink from Zalman. (Can't remember what it's called but it uses heatpipes.)
I'd personally just keep two systems seperate. HTPC using the onboard from an MSI RS480M2 and a gaming system using other louder stuff.
But yeah, the Zalman heatpipe is great, just don't OC anything. Only thing is that it's very bulky, and if you have one of those flat desktop cases, it might not fit, or there might not be a lot of room for airflow (which is essential for anything that's passively cooled). If you just use a regular computer case, it would work fine.
It should be able to play most games at 1024 x 768, and 800 x 600 at worse (except for Doom 3). If it hooked up to a TV, then you don't need super high resolution.
The AC VGA Silencers would be a good option, they are very quiet AND they exhaust the hot air out of the case. Any passive system will require some other fan (PSU) to exhaust the case, which may speed up the RPMs and create even more noise.
