Legend of Zelda. I literally jumped with joy when I finally beat it. I was 5 years old.
My older brother (7 years old at the time) and I took turns and we eventually finished the game.

+1 for Goldeneye. Pretty much justified my reason to purchase an N64.

Close second is Metroid Prime. Then GTA3. I don't think I'll ever forget how fascinated I was with that game.

PC wise...jeez...when you think about all the hours you put into whatever games over the years...makes you feel like you could've acheived a PhD or something.

Need for Speed 3 : Hot Pursuit (can still name all sections of each track)
Starcraft sucked in my whole life for a while (I like to call that while, 'adolescence")
UT99 and its mods, mainly Infiltration

Oldies:

Super Mario World
Streets of Rage

I would have to say Quake 1 on PC is my favorite game of all time. I can't even estimate how much time I spent playing that game. I have such fond memories spanning several years that are just consumed with Quake 1. Especially those summer nights in junior high/high school. I had my own PC in my bedroom, and my parents were awesome and let me stay up as late as I wanted, so I would usually stay up until the sun came up playing Quake, and its several mods (Team Fortress mostly). Man, I really miss those days.

Dark Souls for me - only because it brought back a lot of memories of that first time playing Diablo, trying so hard to make jumps in Castlevania, and that first time I pvp'd in Rune.

I'm replaying FF7 since I got it for $5 on PSN and I have to say that I would be hard pressed to find another game as engrossing and interesting. Just to throw a few out, I could also include CoD4, SW Knights of the Old Republic, Elder Scrolls (console variants, i.e. Morrowind, Oblivion, & Skyrim), Socom 2, and NFL 2k1 are usually on my favorite games lists.

hmmm, the games that have made me go "wow" the most..not necessarily the most played, but the ones I remember the most.

For me, that would probably be:

PC: Call of Duty (1). Installed it, started playing, finished playing in one sitting. I was completely addicted to the sheer level of immersion that the game provided. Sure, it was kind of on rails and not a "thinker" by any means, but the level it made you felt like you WERE in an absolutely whirlwind of combat in some scenes was unprecedented.

Console: Metal Gear Solid. I'd say the 1st, but the 4th is up there too. I loved the way it was shot (I say that like it was a movie :D ), the pace, the story. Sure, a lot of people give it crap for a convoluted storyline and long cutscenes, but it tugged at the movie watcher side of me yet still gave it entertainment. Definitely enjoyed every minute of it (okay, almost every minute).

Seems like the best game ever is always going to be something a person played when they were younger. I remember loving the game Pools of Radiance on my C64. Played it a ton. In retrospect it probably sucked horribly. Goldeneye is the same for me. I played it a ton, split screen 4-player action on a 19" screen seemed absolutely normal. I can't imagine playing anything on a 19" screen now though (without split).
