I would say a buttoned, collared shirt with khakis is generally the "casual business" uniform. You could also wear a polo shirt or sweater, and flat-front khakis or corduroys instead of slacks.
1. Dockers (or equivalent) CLEANED and pressed and worn w/a nice belt
2. Golf shirt or button up shirt, open at collar, no tie
3. Shoes to match belt
4. Nice socks. Matching pants is ideal, but black always works.

Edit:
LOL...when I hit reply there were no other responses.
khakis or slacks, some shirt with a collar, casual shoes (loafers and boots fine). You can also wear a sports coat or blazer if you want. Don't wear a tie though. If you wear a sports coat then a shirt without a collar would also do.
When I worked at (out of business) Flashcom, our business casual allowed for mohawks (in net-ops at least), huge JNCO pants, orange beanies, etc... It was such a wonderful place to work. And then I threw and empty Mickey's beer bottle at their building when I was drunk and vomited in the parking lot at our 'farewell' party...
