What happened to my X800GTO^2?

I'll start from the beginning so I don't miss a thing.

Computer Purchase 9/?/03 or 04:
P4 530@3.0Ghz
1GB DDR2 533 RAM
X300 (Might be a SE)
160GB HD
5.1 Onboard

I Upgraded to a Sapphire X800GTO^2 in March of 2006. I proceeded to unlock the card to 16 pipes (No overclocking though). 2 weeks later I added a Arctic Cooling Rev. 5 to it. About 1 month ago today I upgraded again by adding this: http://www.newegg.com/product/product.asp?item=N82E16820227075.

After all that the comp played fine no problems till all of a sudden about 5 days ago.....

I was playing Guild Wars when all of a sudden my game froze and I had a checkerboard pattern of blue squares on the screen, I could still talk on vent then it froze up.

I did a Hard Restart when I started it up the DOS boot up had grey dots everywhere in rows (This also happened in the BIOS) and when Windows XP booted up it had little green lines going vertically all down the screen.

Once windows had finished loading it would freeze up after the windows XP logo disappeared. After waiting about 5 minutes I would get the BSOD saying the error was in "ati2dvag", which was caused by driver error or the device malfunction (My words). Also it would boot fine in Safe Mode

I put in my old X300 back in and windows booted up fine and played fine, I then updated my drivers to Catalyst 6.9 (From one of the Omega drivers I think).

After waiting about 5 days, I just now tryed to take off my Arctic Cooler Rev. 5 off of it which I couldn't do (Probably because I want doing it correctly) I unscrewed some screws and re-screwed em on. I smelled the card (I couldn't get my nose right on the chip because of the cooler) for any signs of burnt chip smell and there was none. I then slapped it back in the computer and just now booted it up.

I got the same lines and dots from when I started it up before but this time it actually got to the log in screen but the second my mouse cursor appeared it froze up, I then waited 3-5 Minutes and it started moving again. I logged on and proceeded to type this up. I have yet to restart or try anything
 
Yeah when I open up any games I get the blue checkerboard pattern on my screen

*Sigh* Why would it do that after only 5 months (Or so)?
