What exactly is a "Carrier transition"

Like it says. What exactly is a Carrier transition? Is it a physical or logical interface problem, or possibly both?
 
That's where the carrier detect goes from on to off or the other way around. It's an issue with the physical interface (not subinterface). The usual cause that I see for it is that a contractor working with the telephone equipment pulls the wrong card or wire pair.
