What exactly does an IP address Mean?

Just curious, If I have some ones IP address, and I give that ip address to the police, can the police go to the IS Provider and have them reveal the address of that cable modem, who its registered to and etc.?
 
The ISP keeps a record of the IP numbers that were assigned out.

They are not allowed to reveal the identity of the user unless a Judge issued a subpoena ordering them to reveal who used the IP number in question at a certain day and time.
