What does floppy drive failure (40) mean?

I just put together this system with a 8KHA+ but the floppy fails on boot-up. What does code 40 mean?

Most likely you have the cable reversed on the floppy drive. Check this out.

I've also seen that error on a floppy that was outright fried, so if reversing the cable doesn't work, it's probably DOA.
