what does .exe -U mean?

How do I add the "-U" after the ".exe"?

At the command prompt. Start-run-cmd
Then type <path to exe> -U

-U effectively means nothing. It's parsed by the program.

If you need a program to start by a shortcut with parameters, add the parameters after the path quote, like so, in the target box:
"C:\path\to\program\app.exe" -U
