What do the words "neurotic fixations" mean?

Fixations of the neurotic sense.

A guy who is constantly thinking about running around a mall naked smearing peanut butter all over himself would be thought of having neurotic fixations.

In a more serious sense, a person who is constantly thinking of harming someone else in a very bizzare or violent manner would be considered neurotically fixated as well.
