What do the size descriptions of fans exactly mean?

I'm reffering to 60mm, 80mm, 92mm, and 120mm fans. What's the increase of size? The thickness of the fan or the overall length and height? Do 120mm fans fit in the same space where a 80mm fan can go? If not, how do you even put a 120mm fan in your case?
 
I don't know what you mean by diameter but it is actually the length of each side of the fan casing. The fan itself is probably somewhere around 70mm in diameter in a 80mm fan.

they are basically different standard sizes. Rather than making all case fans one size there are a few different sizes to choose from. For the most part all fans are the same size in all three dimenisons as all other in the same size rating. There are a few exceptions, i've seen some slim line fans that are have the thickness.
