Cuba doesn't surprise me. Education there is free and they'll often send med students abroad to study. It's one of the few legitimate ways Cubans can travel. Quality of care is still below first world standards but very high for Latin America.

Well, according to that data, you're almost the same as the U.S. which is 2.3 per 1000 people.

First define what a "doctor" is and can do.

2.3/1000 means what about 400 patients, my guess is that my GP has more than double that, no real issues with appointments etc. and always spends whatever time is needed.
