what causes motion blur?

Is it the video card, monitor or maybe a combo? Like in quake 3 when you spin the mouse its so blurry you can hardly see. I am getting 125 fps though and my monitor refresh rates at 75 and sometimes 85. I have a geforce 2 gts and a samtron 7c monitor, cheap monitor.
