What caused the outage and the data loss?

The last person that asked had done so in a fashion that was perceived as being said with an attitude, though I thought it to have been asked jokingly - the "Fess Up" thread.
So, I shall ask nicely. What did happen? I understand that outages may happen for maintenance, or to implement the subscription system. But why was data lost?
