Western Digital Claims Form

I'm trying to fill out a rebate for this, http://www.newegg.com/Product/Product.asp?Item=N82E16822136073 hard drive. But it says I need a Western Digital Claims Form? What is this and where can it be downloaded?

Where are you seeing that?

I looked at the rebate form (linked on Newegg's site) and all I see is you:

1) Fill out the rebate form.
2) Send a copy of your Newegg invoice

Right underneath the price there's a thing for the $10 rebate. I think that's all you need, I dont see anything that says you need a claims form or whatever.

That's for the current rebate form, mine is effective for the whole Month of March.
