"We Reserve the Right to Refuse Service to Anyone"
NO, they can't refuse to serve you based on the things you mention, but otherwise...you're free game.
Don't smell good? GONE...No shoes, no shirt...NO service...Talk loud? Gone. Have Tourettes? PROBABLY gone...Wear a stupid T-shirt? GONE...

For Race, Religion, Gender, Creed, etc.? No they can't. For a t-shirt they find objectionable, yes they can.

A place of public accommodation cannot refuse service because a person is a member of a protected class.

Example:
http://www.boli.state.or.us/BOLI/CRD/C_Crprotoc.shtml
Discrimination in Public Accommodation
A place of public accommodation is defined in state law as any place that offers the public accommodations, advantages, facilities or privileges, whether in the nature of goods, services, lodging, amusements or otherwise. It is illegal to discriminate in places of public accommodation on the basis of race, sex (including pregnancy), national origin, religion, marital status, physical or mental disability, or age (18 years of age and older).
