Vsync in Metro 2033

Hey I have done some searching and googling but I still can't get vsync to work in Metro 2033. I edited both user.cfg files to enable r_vsync on and tried forcing it on via CCC but still no luck. Am I missing something stupid?

My config:
5850 with cat 10.4
Windows 7 64bit
