Voodoo5 and Opposing Force; won't play now!

Anyone else have any problems with this?

I can play Half-life and Counterstrike just fine, but Opposing force won't let me play.
It worked before I changed to V5.
I have uninstalled it, and reloaded, and I have the latest patches.

I see no reason why it wouldnt work they use the same engine.

It says "Bad surface extents 3408/0"
Then, my desktop is changed to 640 X 480 resolution.
