VoIP home based solutions....Are there any?

Well, the days of free PC to phone calls seem to have passed for good. As such, I was thinking, most of the websites that used to host it would have simply run a frontend java for users with backend gateway control. Well, it seems logical that the backend would be available as a product to buy to run VoIP from home. Hell, Cisco even SUPPORTS VoIP through their routers. So, my questions is this. Are there any software packages which act as gateways for VoIP applications from the desktop? Essentially is there a way to bypass the fees charged by websites? I'm sure there are solutions, but the question is are they affordable enough to warrent them in a home based setting? Anyone have any info or links?

Comcast has been doing some trial runs with VoIP. Do a google search on Comcast VoIP and you'll get some info.
