VOIP hacking?

A coworker uses Comcast for TV, net, and phone (VOIP). He recently broke up with a crazy girlfriend and has said she's able now to see realtime who he calls. She was actually accurate with the information of call duration and numbers called. He says he has a secure router but I haven't been able to look at it to verify. How would she be able to do this remotely and how can he prevent it?
 
One way is if he is using a portable phone that is not encrypted many scanners will receive the older 27Mhz easily and any radio based phone that is not encrypted. I would suggest he change ALL his passwords including call Comcast and change his main password with them. as a start.

That's what I suggested...the password thing. He's in the process of doing that. I don't think anyone was eavesdropping on the conversation because she was asking what he was talking about while on the phone.
 
Err hacking?!
Why don't you check logging in to the comcast WWW site that has stuff like your VOIP phone bill or whatever and see if they don't just display the call records right there. A lot of cell and voip and POTS companies have public tools / web pages to help you track usage of things like call durations, costs, who called, what numbers are dialed out, whatever for billing and account management purposes. Most utilities offer to send your bills online via WWW or via email now.

It seems like if she had shared access to his accounts / passwords then that sort of non-hacking but obvious utility / service thing would be the first thing to check out.

A lot of VOIP services can also be set up to ring the calls on multiple PCs at once and generally let you treat multiple PCs as different extensions of the same line... clearly if it is setup like that then it is conceivable she's just logged in to the same voip account with her PC at the same time as he is and she sees the "line in use with john", "john calling", etc. type stuff.
