Vista Ultimate not adding new things to start menu

About a month ago, Vista stopped adding new items to my start menu. So none of the new games I've installed have been accessible from the start menu. And I just updated Creative Alchemy, and it removed the old version before installing the new version, so now there's no alchemy in my start menu.
