Hey video forum,
I'm usually an OffTopic poster, but I thought I would venture into Video for some help with my system. Just about 2 weeks ago, my near 3-year old build started acting funny. The image on both monitors (dual head setup) will flicker, then put up something like a plaid pattern. Pictures are in the links.

These patterns appear on both monitors.
At first I thought it was just a driver issue: rebooting solved it.
Then it started getting worse, reboots didn't fix it anymore.
So I booted into Safe Mode, uninstalled the ATI driver, and reinstalled it.
That fixed my problem...for a while.
One day I restarted into Safe Mode, and it happened in Safe Mode! Oh noes! It's not apparently a driver issue.
So I rebooted again, and this time it started freaking out in the BIOS. This lead me to think that it is a problem with my video card. What do you all think?
Specs:
ATI 9500 PRO 128MB
NEC Multisync FE991SB and some no-name monitor
Latest ATI drivers
Windows XP Pro
