no one has died on the ice in pro hockey. One guy, I don't remeber his name was severly hurt(broken neck i think) and was alive when he left the building. He died several days later in the hospital.
There was also a close call, 1989, Goal tender Malarchuk of the Buffalo Sabers took a skate to the throat, the blade went under his mask and over the throat gaurd and cut his jugular vien. He bled about 2 pints of blood onto the ice in about 15 seconds. I have the video of it and it is quite amazing to see. It just pumps out like a geyser.

On January 13, 1968, William Masterson received a head injury during a game with the Oakland Seals, and he died 48 hours later at the age of 30. That same year a trophy called the Bill Masterson Memorial Trophy was established by the Professional Hockey Writers' Association, in his memory. After his death, his #19 jersey was officially retired by the Minnesota North Stars.
