If it works fine with another video card, then it isn't cables or the monitors, what PSU are you running?
Maybe the card isn't getting enough power, or maybe it is really dirty power?
Is the molex connector for the video card sharing power with something else like an optical drive?
Is it seated all the way in the AGP port?
Did you overclock it?
Have you tried differnet drivers yet?

I have a ti4600 in right now and it's not doing it...
its like crazy blinking green dead pixels... that are induced by anything moving. It actually looks like I'm o/c too high or something but this card isn't/hasn't been overclocked
