Video card + Fan really loud

I currently have a Geforce 6800 GT with a NV Silencer (5?) attached to it. Lately it's been very loud. Is this a problem? The temperature of my GPU is 58 degrees, and that is pretty much idle I guess. Anything I can do to get rid of the noise?

Same type heatsink for the 6800? I need to replace the thermal gunk then I guess?
