It depends on the card. With most cards out there, plugging right into the header is A-OK. Some card manufacturers use a mini fan socket design on their fans to prevent this. If its a normal fan header, your good to go.
yes, if u plug it directly to the power supply or motherboard without the fan controller it will run at full speed all the time, unless u control it via Qfan or some software based program off the motherboard

I forgot to mention this as well, thanks. If there's a plastic socket that doesn't fit, you should be able to remove it with a pair of pliers and gently wiggle it off.
Or just save yourself the hassle and plug it directly into your motherboard.
