Very weird issue with video card/mouse

So I have a really weird issue that is driving me insane and I have no real idea why its happening but I think the issue is related to my video card for some reason.

I currently have an Asus 290x reference card. When I play Counter-Strike 1.6 (an old game) my mouse will randomly freeze on me and make the usb noise that it got disconnected. Now when this happens two things happen:
1)My computer will lock up and I will get a BSOD
2)I will have one second to alt tab out of the game before my system locks up and it will take me to the desktop. Then I will be forced to wait almost a minute for my mouse to work again.

This only happens when I play CS 1.6 and it happens randomly..sometimes i'll be ingame for less than 2 minutes and it will do this. This does not happen in any of my other games and this started happening when I got this card.

What do you guys think the issue is and is this even fixable? On a sidenote also my fps in CS 1.6 does not stay at max all the time which is weird because even old cards I had 7-8 years ago was able to put out max FPS.
 
What is the BSOD code? It may just be a driver problem if its not happening in other games.
