USB headers on Chaintech CT-SK600

Hi guys,

From the Chaintech manual, I see the following headers
VCC
VCC
D0-
D1-
D0+
D0-
GND
GND


The plugs on my case show:
+5V
+D
-D
GND
GND


My question is where does +5V plug into the mobo? And where would +/-D respectively plug into the mob as well?

For the 2nd USB, there's on 1 GND - what's the deal with that?
