USB/Firewire adapter card

Does anyone know of a good adapter card to add firewire and USB 2.0 to and older computer? Specifically, I need one with two USB 2.0 headers on the card. I've got a multi-format card reader with firewire and USB 2.0 ports on it but it has two separate USB cables (one for the card reader portion and one for the front panel USB port). To make both portions work at the same time, I need a card with two USB 2.0 headers. And no, my motherboard doesn't have any USB headers on it to plug into, just the two ports on the back of the board.
