UPC code for an eVGA graphics card.

It's a sticker on the back of the box, but it's surrounded by the part number and the serial number.
eg: http://evga.com/register/serial_number_label.gif

Do I just send in the UPC code part, or the entire sticker, for the rebate? Anybody with any experience in this?

Send the entire sticker.

Yes, you send the entire sticker, but be sure to make yourself a copy or two (of everything you send) and only send the original of anything (receipt, UPS, etc.) if the rebate form says you must send the original.
