Have you installed all the motherboard drivers? Windows sometimes has trouble autodetecting USB controllers, especially if they're USB 2.0 ones. What motherboard do you have and what operating system are you running?

A USB controller is just what it sounds like. It is a device that allows one to connect USB devices to your computer. Don't worry about it unless you have a USB device.
