Uh oh...is my X800XT PE dying? With weird screenshot from Civilization 4

I purchased an ATI Radeon X800XTPE (built by ATI brand) back in the summer of 2005. It's worked fine since then, but lately it's been acting a little strange. In the past few months, I will be playing a game like Civ 4 or Europa Universalis 3 and all of a sudden everything will just freeze and I have to do a hard reset. Playing Civ 4 in windowed mode seems to mostly solve this though. I tried updating my drivers a few weeks ago but the problem persists. Today, I fired up Civ 4 and I was greeted by this:

ATI Tool is reporting unusually high core temps, close to 100C when I have the fuzzy cube on screen to test for artifacts. This was not the case when I got the card, it rarely got above 60C.

What troubleshooting steps should I be taking? Do I need to start looking for a new card?

Whoa...Is your cooler working? Air blast your fan to ensure there is no dust built up. Is the fan even coming on?
Looks like you may have to get a newer one...Santa!

My x850 XTPE did that with an inadequate power supply (Enermax 350w). I swapped in a 430W Antec and it went away. I could also get it to stop by underclocking the card, which supports the idea that the PSU wasn't sufficient.

The fan definitely is responsive- I hear it power up whenever I am playing games, maybe the contact is bad? The PSU is definitely not an issue, it's an Antec 500 watt.

If the card is in fact a POS, it looks like I got really lucky and got a 3 year warranty just days before ATI switched it to one year.
