Never really had to do this, but I might have the opportunity very soon! (New job lined up is looking very good)

Is it customary to give your two-weeks notice in written form or is a verbal notice just as good? I've worked at my current job about 9 months and not sure how to approach the situation when the time comes. Also, they don't know I am planning on leaving soon.

depends on how well you get along with your boss. If your cool with him/her tell them to their face. It shows respect....IMHO.
BUT you can't go wrong written either...

just a simple paragraph, a couple of sentences. you don't have to explain why you're leaving.

i gave mine last week

if you're in a sensitive line of work, you may want to take home your personal belongings before resigning
i verbally resigned last tuesday
gave my boss a letter wednesday morning (www.i-resign.com has great templates)
2 weeks became 2-1/2 weeks, possibly more now if i get a second offer.
