"Turn your head and cough"

When you cough your intestines pop out and then retreat. By placing some fingers along the wall you can feel if they pop out.

I know the purpose behind it, but how does it actually work? How does one feel the intestines by cupping the balls?

Because if there is a hernia (a weakness in the abdominal wall muscles) the intestines can poke through the the weakness. A common place for a hernia is in the 'inguinal' region - and in that case, the natural place for the piece of intestine to fall into is the scrotum.

Coughing, raises the pressure in the abdomen, by tensing up the muscles. The increased pressure pushes any intestines out further through a weakness, so it is possible to feel them bulge in response to a cough.
 
It's usually more than cupping. You poke your fingers up into the scrotum so you're right up there against teh wall.

yeah my female doctor now gives me a disgusted look when I go in for my daily visit for hernia check. 🙁
 
Of course it doesn't work. They've just been doing it for hundreds of years because they want to fondle your balls.
