Trouble with DUN in XP-Pro

Whenever I reboot and then try to connect to the internet the Username and Password boxes are blank.

I have set them up using Internet Options/Connections/Settings and
I have set them up using Control Panel/Phone and Modem Options.

Both end up with the same results.

Any ideas on what is going on and how to fix it?
