oh yea, make sure it's not still bleeding. If it's bleeding, wait a day or a few for it to heal. Keep it clean. After a while you'll get annoyed by the feeling of the toenail while walking and you'll want to pull it off. Hopefully by then, the skin will have soften enough and then rip. (I am assume that it is not hanging by much. Don't rip it if it's still attached to a good amount of skin.)

Take it off. It will fall off anyway.

I lost mine nearly a year ago and it's only half grown back. It will take awhile. Be patient. Try to keep it hydrated so it doesn't in-grow.

It usually begins with the toenail falling off. Then other things fall off around the waist area as the disease progresses upwards.
