I recently thouhgt about this and it really depends...

is the roll above or below eye level? If you are in a public stall where the roll is higher up...its easier if it is UNDER the roll....however at home...where the roll is more likely to be lower down...its easier for it to be OVER the top

don't have a toilet paper holder thing in my bathroom (well, we do, but it's in a horrible position). we just leave the roll sitting on the ledge of the window that's right next to the toilet.

ghetto but convenient.
