I've been handing in the same self-evaluation for the past 4 years and no one's noticed...

Geez... at least I take a few minutes to update the dates and the version numbers of the products I test/support before sending it in!

You know, just in case there is a chance that my boss actually decides to read it this year :)

In the OP's case, he should ask for a example of a completed appraisal form just to get idea what HR wants. And then plagiarize the crap out of it, changing project names and dates as needed.

The only reason why I would rather work for myself. We have to do the plans at the beginning of the year and results at the end. The cool part is we just copy and paste what our managers give us because it had to be in line with their managers. End of year we just paste achievements. Not a ton of bs that we have to wordsmith ourselves.

I don't remember it being quite that easy... there was also a section that you had to fill out that said what classes you wanted to take that year to improve your skills. That also became a cut and paste job as well after a few years of them not getting the classes approved due to various lame assed budget issues.

i was about to write that i dont fit in and such, but i deleted most of that, half filled it and sent it to HR asking for advice on how to complete it. this after asking my colleagues how they answered and most of them telling me they practically handed it in blank.
even though i had a great first 11 months, i made 2 mistakes in october, and while it's debatable whether they are real mistakes or different interpretations of teh data, i had a bad score, so i couldn't write how awesome i am and such.

not planning to stay here for long anyway, just until march would be cool.

That'a boy you did fine. Don't take it personally. Most of these types of hell holes are bound to go out of business eventually its just a matter of time.
