With motherboards, I've gotten some with and some without the accessories, but obviously those accessories aren't critical for the function of the motherboard.
That would be my main reason to hesitate.
I didn't say I was buying this...

.....and the possibility of getting just the cooler without the plate I need is why. Of course, I did manage to get in on the Cooler Guys' Scythe Andy Samurai Master heatsink when it was on sale for $19.99.....
I've bought open box from Newegg before, and as you said, it can range from looking just like brand new and never opened to only the main part with none of the accessories. It's a crap shoot at best.
But, I do take my chances if I don't need any of the accessories, like that Gigabyte socket 754 motherboard a couple of days ago for $19. Don't need a thing except the motherboard....even the rear I/O plate can be replicated out of an old one I've got laying around and all the drivers can be downloaded from Gigabyte very easily.
Same with cpus....either they work or they don't. What accessories do you need? But with a universal heatsink, you're taking a chance...but at least you can return it. A better bet is a socket-specific heatsink.....usually all necessary parts are attached.
But I just report what I've found....don't pass judgement on whether it's worthwhile to buy or not...up to the end purchaser.
