Theoretical Question About a DVD

Let's say you were burning copies of some movies. All usually goes fine. No coasters, no problems. Burn and return.
But, for some unknown reason, using the same hardward and software, one movie title continually comes up with errors during the burn. Has this happened to anyone? What might be the cause of such an event?

Under the presumption this was for a backup purpose only...(right??) it could be the media itself such as scratches or some other issue with it or a newer copyright protection scheme.
