the word '[SPAM]' shows up in all outgoing outlook messages

I can't figure this one out. She isn't computer literate to even attempt to setup anything but every message she sends from Outlook has [SPAM] as the first word in the subject. I sent webbased email from her account and didn't receive it so I figure that narrows it down from the ISP to her Outlook. When I was home I checked every imaginable setting and I didn't see any rules laid out.

Anyone seen this before?

Does she use a Security Program, like Norton Anti-Spam?
Have you checked the machine for spyware?

No anti-spam programs.

She uses avg for antivirus and adaware. Everything comes up clean.

Did she enable any Junk Email filtering? (Tools | Options | Junk Email button)

There is a junk email folder and my personal email address is in the list of safe emails but there is nothing else. Plus, this is only applied to sent emails which makes it even stranger.

Personally, I think it would come down to 2 options.

1. ISP is doing it
2. Antispamware on the computer

I have eliminated the ISP thing by using her webmail account on her internet connection and I never installed any antispamware stuff and I didn't see any in the programs list or as a process running.

At my previous job I've seen the '[SPAM]' tag on incoming Emails on a GoDaddy hosted account but not outgoing. This happened on all of our systems so I don't think it was specific to any settings in Outlook. Also, it was the same on Webmail so it's different than what you're seeing. Really strange...
